Does the sum of the Adjusted Gross Income (AGI) on the last federal tax returns of everyone in your household fall above or below three times the Poverty Guideline number that matches the number of people in your household? Or, do you receive financial assistance (not medical assistance) from the Ohio Jobs & Family Services Agency or a county agency?
For example, if 2 adults and 3 children are in your household, then you have 5 people in your household. Find the row for Five People and multiply the Poverty Guideline by 3. Tell us if the sum of the AGIs of everyone in your household is above or below this number.
